ONLINE CONSENT FOR RESEARCH PARTICIPATION
NOTE: Participation in this research is open to faculty and academic staff members who have teaching responsibilities.
By clicking on the "AGREE" button below, I hereby give my consent to participate as a subject in an investigation conducted by Michele Jacobsen into the Teaching and Learning with Technology in Higher Education as part of her dissertation research.
I understand that:
- My participation in this research will involve completing a survey about patterns and history of computer use, self-efficacy items, and demographic information. The survey also collects information about changing classrooms, incentives, and barriers to integrating technology into teaching and learning. The entire survey will take approximately 30 to 45 minutes.
- My participation in this study is voluntary, and may be terminated at any time by my request. Participation in this project and/or withdrawal from this project will not adversely affect me in any way.
- This study will not involve any greater risks than those ordinarily occurring in daily life. I understand that it is not possible to identify all potential risks in any procedure, but that all reasonable safeguards have been taken to minimize potential risks.
- Responses will be obtained anonymously and kept in strictest confidence, and the results of this project will be coded in such a way that my identity will not be attached in any way to the final data that I produce.
- The results of this research may be published or reported to government agencies, funding agencies, or scientific groups, but that my name will not be associated in any way with any published results. I understand that only GROUP data will be reported in any published results.
